Information We Collect During Registration or During use of Website:
Required information we collect during the registration process includes without limitation your name, mailing address, phone number and e-mail address. We also collect billing information such as credit card number, expiration date and security code, and billing address.
When you use the GoPHR Web site, GoPHR automatically collect and stores: the name of the domain and host from which you access the Internet; the unique Internet protocol (IP) address of the computer you are using; the browser software you use and your operating system; the date and time you access our sites; and the Internet address of the site from which you linked directly to our site(s).
Information Collected as You Use the Software Application:
We also collect and store information that you voluntarily provide through the day to day use of your GoPHR profile once you have registered. This information is provided by you or your physicians or other authorized users, and we store this information in a private database to ensure security. Any time you edit or modify your information in GoPHR, we will send that information to our secure server Secure Socket Layer (“SSL”) encryption technology. SSL encryption is an industry-standard technology and is used by financial institutions or the equivalent thereof to protect your data. You may access your information at any time as long as your registration with GoPHR is current. If you have canceled your registration with GoPHR, we will continue to securely store your information for up to six (6) months, during which time you will have access to view or save that information, but you will not have the ability to edit. At the time of cancelation, you will have the option to opt out of GoPHR saving your personal health information for that six month period by providing written notice to GoPHR within seven (7) calendar days after such cancellation. Failure to provide such written notice will be deemed an election not to opt out, in which case GoPHR will save and store your information accordingly. If you opt out of this service, GoPHR will dispose of such information in accordance with standards no less than the HIPAA standards and regulations for disposal of electronic and/or paper personal health information records.
GoPHR may use information we collect to:
- Create your personal GoPHR account
- Send you a confirmation of a successful order
- Respond to customer service requests and assist you with customer service requests
- Supply the Administration for your account
- Improve the GoPHR Website and offer free enhancements to our existing software
- Develop and improve additional websites connected or otherwise affiliated with the GoPHR Website
- Conduct demographic and marketing research and analysis
We may post customer testimonials on the GoPHR Website which may contain personally identifiable information. We do obtain the customer’s consent via e-mail prior to posting the testimonial to post their name along with their testimonial.
Disclosure of your information
Except as provided in the first paragraph of this section, GoPHR will not disclose to unaffiliated companies any Personal Information that you store in GoPHR. You are the only authorized user on your account unless You personally authorize additional users for which You take responsibility. GoPHR will take all reasonably necessary steps to ensure that no unauthorized person shall have access to your GoPHR password or account. However, it is your sole responsibility to (1) control the dissemination and use of activation codes and passwords; (2) authorize, monitor and control access to and use of your GoPHR account and password; (3) promptly inform GoPHR of any need to deactivate a password. You voluntarily and unequivocally grant GoPHR and all other persons or entities involved in the operation of the GoPHR Website and its services and products the right to receive, transmit, monitor, retrieve, store and use your Personal Information, including without limitation information which may be privileged and confidential under applicable state and federal laws, in connection with the operation of GoPHR. GoPHR cannot and does not assume any responsibility or liability for any information you submit to GoPHR or your or any third parties’ use or misuse of information transmitted or received using GoPHR.
Information Collected about Children and Website Usage by Children
The GoPHR Website is not intended for use by individuals under the age of 14 unless parental consent is provided. The GoPHR Website is a storage facility and only contains information about children to the extent you, the user, provides that information. It is exclusively your responsibility to protect your passwords and account information to prevent such use. Protecting passwords and other account information is particularly important if information about children under the age of 14 is stored on the Web site.
How GoPHR Handles Privacy Internally

GoPHR will never request your password via e-mail or telephone. Do not respond to any e-mail requesting password information from you. Any such e-mail request should be regarded as phishing and we ask that you report any such e-mails to our customer service center as soon as they are received.
Please be aware that GoPHR is not responsible for the privacy practices of any other companies or affiliates or websites that may be linked to either the GoPHR Website or our GoPHR software application. We encourage you to read the privacy statements of each and every web site that collects personal information from you.
You can always contact us in order to (1) delete your Personal Information from our systems, or (2) update the Personal Information that you have provided to us.
