GoPHR takes your privacy seriously. That’s why, even though PHR companies are not subject to follow the rules and regulations required by HIPAA, GoPHR has created a safe online health management system that not only meets but exceeds those regulations, with the understanding that nothing shall be interpreted as an acknowledgment or agreement that GoPHR is actually or potentially liable or otherwise responsible under HIPAA or any other law that is not applicable to PHR companies. We want our users to feel comfortable and safe at all times while using our software application. Please read our Privacy Policy so that you will understand how we have chosen to protect you and your private health information.

Our privacy policy covers both the use of our website www.gophrit.com (the “GoPHR Website”) and our personal health records software, GoPHR. Information We Collect During Registration or During use of Website:

Required information we collect during the registration process includes without limitation your name, mailing address, phone number and e-mail address. We also collect billing information such as credit card number, expiration date and security code, and billing address.

When you use the GoPHR Web site, GoPHR automatically collect and stores: the name of the domain and host from which you access the Internet; the unique Internet protocol (IP) address of the computer you are using; the browser software you use and your operating system; the date and time you access our sites; and the Internet address of the site from which you linked directly to our site(s).

GoPHR may collect and utilize this information in its sole discretion to conduct system administration and site analysis. To maintain visitor anonymity, GoPHR does not associate IP addresses with records containing personal information. However, GoPHR will use IP addresses to identify any visitors who refuse to comply with GoPHR’s rules or terms of use.

Information Collected as You Use the Software Application:

We also collect and store information that you voluntarily provide through the day to day use of your GoPHR profile once you have registered. This information is provided by you or your physicians or other authorized users, and we store this information in a private database to ensure security. Any time you edit or modify your information in GoPHR, we will send that information to our secure server Secure Socket Layer (“SSL”) encryption technology. SSL encryption is an industry-standard technology and is used by financial institutions or the equivalent thereof to protect your data. You may access your information at any time as long as your registration with GoPHR is current. If you have canceled your registration with GoPHR, we will continue to securely store your information for up to six (6) months, during which time you will have access to view or save that information, but you will not have the ability to edit. At the time of cancelation, you will have the option to opt out of GoPHR saving your personal health information for that six month period by providing written notice to GoPHR within seven (7) calendar days after such cancellation. Failure to provide such written notice will be deemed an election not to opt out, in which case GoPHR will save and store your information accordingly. If you opt out of this service, GoPHR will dispose of such information in accordance with standards no less than the HIPAA standards and regulations for disposal of electronic and/or paper personal health information records.

GoPHR may use information we collect to:

  • Create your personal GoPHR account
  • Send you a confirmation of a successful order
  • Respond to customer service requests and assist you with customer service requests
  • Supply the Administration for your account
  • Improve the GoPHR Website and offer free enhancements to our existing software
  • Develop and improve additional websites connected or otherwise affiliated with the GoPHR Website
  • Conduct demographic and marketing research and analysis

We may also collect information about you and your use of our site through the use of Cookies. “Cookies” are small computer files that we transfer to your computer’s hard drive, which allow us to know how often someone visits our site and the activities they conduct while on our site. This information helps us dynamically generate advertising and content on web pages specifically designed for you and also allows us to statistically monitor how many people are using our site and for what purposes. Sponsors and advertisers may also use Cookies when you select their advertisement, and GoPHR is not responsible for such use by sponsors and advertisers; we are unable to control their use of Cookies or how they manage the information they gather. We do/do not link the information we store in cookies to any personally identifiable information you submit while on our site.

Customer Testimonials

We may post customer testimonials on the GoPHR Website which may contain personally identifiable information. We do obtain the customer’s consent via e-mail prior to posting the testimonial to post their name along with their testimonial.

Disclosure of your information

Except as set forth in this paragraph, GoPHR will not disclose to unaffiliated companies, persons or other entities any information it gathers from you which could be used to identify or contact you (“Personal Information”). For the purposes of this Privacy Policy, Personal Information does not include (1) information that becomes generally available to the public or any third parties through no act or omission of GoPHR; (2) information acquired at any time from a source other than you, but only if the source was not prohibited from disclosing such information; and (3) information required to be disclosed in order to comply with any applicable law, order, regulation or ruling. We do not sell your Personal Information to third parties. Any Personal Information provided to persons or entities affiliated with GoPHR will be treated in accordance with the terms of this Privacy Policy, unless you are otherwise notified in writing (which may include e-mail). In the following limited circumstances we will consider, and may release, Personal Information to third parties without prior notice or consent: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order or ruling; or (2) in special cases or under exigent circumstances, such as a physical threat to you or others. GoPHR may release Personal Information at any time upon your prior written approval (which may include e-mail), such approval not to be unreasonably withheld, conditioned or delayed. GoPHR may at any time, without prior notice and consent, provide information about you that does not allow you to be identified or contacted (“Aggregate Information”) to third parties. For example, we might inform third parties regarding the number of users of our site and the generic activities they conduct while on our site or we may provide demographic or usage information to a corporate client concerning the registrations they provided to employees or affiliates.

Except as provided in the first paragraph of this section, GoPHR will not disclose to unaffiliated companies any Personal Information that you store in GoPHR. You are the only authorized user on your account unless You personally authorize additional users for which You take responsibility. GoPHR will take all reasonably necessary steps to ensure that no unauthorized person shall have access to your GoPHR password or account. However, it is your sole responsibility to (1) control the dissemination and use of activation codes and passwords; (2) authorize, monitor and control access to and use of your GoPHR account and password; (3) promptly inform GoPHR of any need to deactivate a password. You voluntarily and unequivocally grant GoPHR and all other persons or entities involved in the operation of the GoPHR Website and its services and products the right to receive, transmit, monitor, retrieve, store and use your Personal Information, including without limitation information which may be privileged and confidential under applicable state and federal laws, in connection with the operation of GoPHR. GoPHR cannot and does not assume any responsibility or liability for any information you submit to GoPHR or your or any third parties’ use or misuse of information transmitted or received using GoPHR.

Information Collected about Children and Website Usage by Children


The GoPHR Website is not intended for use by individuals under the age of 14 unless parental consent is provided. The GoPHR Website is a storage facility and only contains information about children to the extent you, the user, provides that information. It is exclusively your responsibility to protect your passwords and account information to prevent such use. Protecting passwords and other account information is particularly important if information about children under the age of 14 is stored on the Web site.

How GoPHR Handles Privacy Internally

GoPHR uses SSL technology to help ensure the integrity and privacy of the Personal Information you provide to us via the Internet. As an additional security measure, your Personal Information is also kept physically separate on a separate server, where your password is encrypted and stored on a database. In our administration, only authorized GoPHR employees are permitted to access your personal information. All GoPHR employees must abide by our Privacy Policy and those who violate our Privacy Policy are subject to disciplinary action. All authorized GoPHR representatives have received a certificate of HIPAA Compliancy and are trained to understand all HIPAA rules and regulations involving personal health information, again with the understanding that nothing shall be interpreted as an acknowledgment or agreement that GoPHR is actually or potentially liable or otherwise responsible under HIPAA or any other law that is not applicable to PHR companies.

Your Privacy Choices

GoPHR will never request your password via e-mail or telephone. Do not respond to any e-mail requesting password information from you. Any such e-mail request should be regarded as phishing and we ask that you report any such e-mails to our customer service center as soon as they are received.

Please be aware that GoPHR is not responsible for the privacy practices of any other companies or affiliates or websites that may be linked to either the GoPHR Website or our GoPHR software application. We encourage you to read the privacy statements of each and every web site that collects personal information from you.

The GoPHR Website includes Widgets, which are interactive mini-programs that run on our site to provide specific services from another company (e.g. displaying the news, opinions, music, etc). Personal information, such as your e-mail address, may be collected through the Widgets. Cookies may also be set by the Widgets to enable them to function properly. Information collected by each Widget is governed by the privacy policy of the company that created it.

You can always contact us in order to (1) delete your Personal Information from our systems, or (2) update the Personal Information that you have provided to us. Changes in this Privacy Statement

GoPHR may change this Privacy Policy at any time without prior notice by posting revisions to the GoPHR Website. Your use of the GoPHR Website constitutes acceptance of the provisions of this Privacy Policy, and your continued usage after such changes are posted constitutes acceptance of each accordingly revised Privacy Policy. If you do not agree to the terms of this Privacy Policy or any revised Privacy Policy, please do not register for a GoPHR account or please cancel your GoPHR account immediately and refrain from any further attempts to access or otherwise use the GoPHR Website, as you will be bound to the most recently revised Privacy Policy if you are using GoPHR. If we decide to change our privacy policy, we will post those changes to this privacy statement, the home page, and other places we deem appropriate so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. We reserve the right to modify this privacy statement at any time, so please review it frequently. If we make material changes to this policy, we will notify you by whichever means we deem to be appropriate in our sole and absolute discretion, including without limitation here, by e-mail, or by means of a notice on our home page.
